Students will need:

Emergency Contact Information, including health care provider contact information.

​

Completed Health Inventory form, to be completed by both the parent and health care provider.

​

Immunization records or a signed State of Maryland Exemption Certificate required for personal, religious or medical exemptions to school immunization requirements.

​

Parents will receive additional forms to be completed and returned as soon as possible.

​

There will be a non-refundable registration fee of $50.00 and a 1 week advanced tuition to be paid at the time of enrollment.
